OFFICE HOURS:

Monday through Friday
9:00 am to 12:00 noon
2:00 pm to 5:00 pm

When the office is closed our answering service will respond to your calls through our usual telephone number, 944-0166. The pulmonologist on call is available for emergencies or urgent problems.

APPOINTMENTS:

We would appreciate your arrival 15 minutes prior to your appointment time for the first consultation visit in order to make sure all your paperwork is completed. In meeting our goal of providing high quality care and convenience for our patients, please note that your appointment time may be released if you arrive past your scheduled time.

CANCELLATIONS:

To accommodate the needs of all our patients, we require 48 business hours notice of cancellation of your appointment. If we do not receive notification you will be assessed a $50 no show or cancellation fee. If you have questions regarding this policy, please speak with the Office Manager.

COMING TO THE OFFICE:

Questionnaire and Records:
As a new patient we ask that you bring the following documents with you in order for the physicians to fully assess your needs.

  1. Reports of diagnostic testing that relates to your visit. X-Rays, CT’s, Sleep Studies, Labs
  2. A list of current medications.
  3. A referral as needed by your insurance.
  4. Completed new patient packets (enclosed forms).
  5. Insurance cards.
  6. Picture ID.

The Medical History:
The physician will ask questions about your current problems as well as your past medical history. Give some thought before hand so that you will not omit important facts that the physician should know. Please bring all of your current medications or a listing of them indicating the dose and frequency.

The Examination/Consultation:
Time spent with the doctor will vary with each patient and his or her unique medical needs. However, please keep in mind that the consultation includes time not actually spent with you. This non face-to-face time consists of obtaining and reviewing your X-rays and/or, Cat Scans, laboratory tests, sleep studies and reporting results to your referring physician.

X-ray and Laboratory Work:
The physician may request relevant tests to aid in diagnosing your problem. Staff will assist you in scheduling these tests in an outside facility.

PRESCRIPTION REFILLS

Allow 48 business hours for staff to address your refill needs. When making a request, please have the pharmacy name and fax number.

TELEPHONE CALLS

Between the hours of 9:00 – 12:00 and 1:30 to 5:00, the office will make every attempt to answer your calls personally. In the event that we are unable to answer your call, please leave a message and your call will be returned within the hour, or in the event it is after 4:30 the next business day.

EMERGENCY CARE

A pulmonologist is available 24 hours a day to deal with urgent problems. During normal business hours, the patient care coordinator will be available to assist you in determining how to schedule your appointment. If your emergency occurs after hours the answering service will immediately contact the on-call physician to handle the emergency. If the situation is life threatening, call 911.

Our physicians are on staff at John Muir Medical Center. In the event you need hospital care, please ask for the Respiratory Medical Group physician on call.

PAYMENT POLICY / INSURANCE

Payment of your co-pay or deductible is required at the time of services. If your insurance cannot be verified, we will request payment at the time of your visit. For your convenience, we accept MasterCard, Visa, personal checks, money orders or cash.

Respiratory Medical Group participates with most insurance plans and will file a claim for services on your behalf. In the event you have secondary insurance we will do a one time courtesy billing. However, we do not bill secondary insurances for co-pays.
